OBTAINING A CERTIFICATE OF INSURANCE/PROOF OF INSURANCE: If you are asked to produce “Proof of Insurance” or a “Certificate of Insurance” while on University business, please complete a request for certificate of insurance form.  This form must be submitted by a McMaster employee.

Note:  The top portion of the form is the information of the organization requesting the certificate.

Once completed submit form to Nina Cavalho, Sr. Insurance Specialist, Enterprise Risk via email: carvas2@mcmaster.ca

CERTIFICATES FOR NEXT YEAR:  If you are requesting a Certificate of Insurance for the following year, please submit your request no earlier than the end of November of the current year.

Note:  Certificates of Insurance can only be issued for one year at time (January – December).

If insurance coverage is required for the transport of McMaster University property in transit please complete the attached spreadsheet and submit to Nina Cavalho, Sr. Insurance Specialist, Enterprise Risk via email: carvas2@mcmaster.ca.  Please note a chartfield string is required to chargeback to the department for insurance coverage.  Once received you will be provided with a estimated insurance premium cost.

Note:  Insurance premium charges are charged back at year end.

Purchasing/Leasing or Renting (30 plus days) a vehicle for McMaster business?Provide the following information to Stacie Cameron at venns@mcmaster.ca:

• Model
• VIN#
• Licence Plate#
• Year
• Colour
• Chartfield String
• Length of rental

If drivers have not been added please also include name and driver’s licence number of driver.

When renting a vehicle for McMaster business and under 30 days please ensure additional insurance coverage is purchased from the rental company.
