If you are enrolled in the Desjardins Group RRSP plan, please also ensure you update your information with Desjardins directly. Log into the Desjardins website here and complete the necessary forms under “My statements and documents” –> “Forms and documents” –> “General Forms”. To update your marital status, address, and/or name, complete the “Notice of Change to Plan Member Record” Form. To update your beneficiary, complete the “Beneficiary designation” form.
As of January 1, 2012 the Bill 133 – Asset Division on Marriage Breakdown identified that a division of pension assets cannot be processed unless the Plan Member (or the Member’s spouse) submits a request for a Family Law Value to the Plan Administrator. The forms are administered by the Financial Services Regulatory Authority (FSRA) of Ontario, formerly Financial Services Commission of Ontario (FSCO). The applicable forms and additional information can be found here. The Plan Information to Complete the Family Law Value Form can be found here. To proceed with this request please submit the FSRA forms and prescribed documents to your HR Representative.

Extended Health and Dental Enrollment Form – To add a new dependent child to your benefits, please complete and submit this form to hr.mcmaster@mcmaster.ca. Coverage can be retroactive if we are notified within 31 days of the date of change. Be sure to include all eligible dependents on this as we only keep the most recent form submitted on file. Review and Update Beneficiary Information: If you’d like to make an update to your beneficiary information, please complete the applicable forms below and submit this form to hr.mcmaster@mcmaster.ca

McMaster University is fortunate to have on-campus daycare services for faculty, staff and students. McMaster Children’s Centre is a licensed, non-profit, self-supporting corporation that is administered by a director and Board of Directors. McMaster Children’s Centre opened its doors in 1975 and has continued to serve the community to this day. They are currently licensed for 63 children, ranging in ages from 18 months to 5 years.
